Abstract

Severe hypercarbia can develop for unidirectional valve malfunction and may go undetected during anesthesia. A 29-year-old male patient with history of bronchial asthma underwent tonsillectomy and septorhinoplasty due to chronic tonsillitis, depressed bulbous, and deviated septum of nose. Anesthesia was induced with thipental-vecuronium and maintained with enflurane-N2O-O2 in a semiclosed circle system. We experienced a case of severe hypercarbia from replacement with an incorrect expiratory valve.
